Equipment Reservation Form

Reservation Policy

Please read the equipment reservation policy and submit the form one week in advance.

  • Equipment may be reserved/checked out by faculty or staff only.
  • Reservations must be made at least five days in advance.
  • The maximum loan period is two weeks.
  • Any additional software loaded or system modifications must be approved by the Director of Academic Computing
  • Any data files saved on the hard drive must be deleted prior to return.
  • All technical problems must be reported to Director of Academic Computer Services.
  • **Setups will be done no earlier than 5 minutes before the class starts
  • **All professors are required to inform Academic Computing Center in case of a cancellation of the reservation request or any change in the duration of the equipment usage (call x4022 in case your class finishes earlier. Do not leave equipment unattended).
